Employee Exit Questionnaire Guide


Completing this questionnaire is voluntary (but encouraged). It may be completed on your own or with the assistance of the Human Resources Department. You do not need to answer those questions which do not apply to you.

The exit questionnaires are considered strictly confidential. The information is collected for the purpose of identifying ways to improve the working environment of current and future Society employees and, as such, your participation in this process is valuable and important in helping us achieve these goals.
If you have any questions regarding the questionnaire or the process, please contact the Manager of Human Resources.

Completion Guide – Exit Questionnaire
Thank you for agreeing to complete this exit questionnaire. This guide is designed to answer questions you may have and provide you with instructions for completing the questionnaire. The Manager of Human Resources can provide you with further information if needed.

Why are departing employees asked to complete an exit questionnaire?

Exit questionnaires allow the society to gather information on specific issues in a systematic way. The information collected can identify areas needing change as well as employee training and development needs.
Exit questionnaires also provide an opportunity for departing employees to provide feedback on issues that they may not have felt comfortable discussing during their employment.

Who will see my completed questionnaire?

Unless you give permission, the only people who will see your completed questionnaire is the Manager of Human Resources.

What do you mean by “May we provide a copy of this Exit Questionnaire to
your Supervisor”?


As mentioned, the Manager of Human Resources will have access to your information. It is up to you to decide if you want your Supervisor be able to see your Exit Questionnaire. If you say “No”, the exit questionnaire will not be shared with your program or Supervisor.

What is done with information collected?

Exit questionnaire information will be summarized at the end of each fiscal year and will become part of the annual Human Resources report. No employee names or identifying information will be included in this report.

The Society will use the information gathered as part of their strategic planning and to address specific issues such as retention.

Programs will use information to identify the need for changes in their human resource practices and to identify training and development needs of supervisors and other employees.

Completion Guidelines:

• Some questions may not apply to everyone, if you don’t believe the question applies to you, write N/A (not applicable).
• The questionnaire is designed to seek feedback on areas departing employees believe could be improved or areas to which they have a positive regard.
• If you are unsure of the meaning of a particular question on the exit questionnaire, please discuss this with the Manager of Human Resources.
